- About the facility Brian Center Health & Rehab -Weaverville
Memory care facilities provide housing, care, and therapies for seniors who have Alzheimer’s disease or other forms of dementia in an environment designed to reduce confusion and prevent wandering. We are paid by our participating communities, therefore our service is offered at no charge to families. With the lawsuit, Ms. James seeks to prevent new resident admission, install monitors to oversee the facility's operations and finances, and ban existing or hidden owners from their roles. Among the examples noted in the lawsuit, a diabetic man was given a wheelchair without footrests, forcing him to drag his feet on the floor — leading to sores. Part of his toe had to be amputated at the hospital as a result of the infections, and not long after he returned to the center, he died, the investigation found.

I found this place to be very loving and helpful. Staff are all wonderful and will help at any time of need. My family member loves the set up and the things to do, she has fun all day. Great place the administrator is very nice and welcoming. Brian Center Nursing Care - St Andrews is a senior living provider in Columbia, South Carolina that offers residents Nursing Homes.

I wish we had never decided to send my Grandmother to this facility. Communication is horrible, especially during covid. A phone that's probably not cleaned between patients because they pick it up and carry straight to the other patient. My Grandmother wore the the same outfit twice in a row because they can't find her clothes. My Mom was talked to ugly on the phone when trying to find out where my Grandmother's clothes were. My Grandmother's doctor was NEVER consulted prior to deciding not to take her to a critical appointment.

He would later be admitted to the hospital for severe malnutrition, dehydration, a worsening pressure injury and a bone infection in his right foot. During the first few months of the pandemic, from March to June of 2020 when nursing home staffing across the country was stretched beyond thin, 166 Cold Spring Hills residents died, 98 of which were from COVID. The facility failed to report 51 of those deaths to the state's DOH, the lawsuit stated, a fraudulent underreporting by more than 50 percent. Please note that visitors will be required to submit a negative COVID-19 test , use PPE as well as maintain social distancing during visits at the facility.

I would highly recommend them and wish to thank them for their service to him. Anything you can share will help other families choose the perfect community. This was the most unclean facility I have ever been in. The management does not know what they are doing and my grandparent got sent to the wrong facility to have a procedure done outside of the brian center.

The person looking after him was not told when the man died, or even was informed of his condition after going back to the facility. More than $15 million in phony rent payments were mad to Cold Spring Realty — which just so happens to be owned by the same people who operate the nursing home, the investigation found. There was also more than $5 million allegedly paid for what was said to be "consulting."
It is worth mentioning that Medicare pays for what they deem as 'medically necessary' skilled nursing care. My family member was in a facility in Elizabeth City and was not cared for. My family member had a wound from the other facility and after being in the Brain center for a week it was so much better. The room my friend is in is dirty and smells.I feel she is not getting the proper rehab.for her stroke. The help my mother alot since she doesn't remember alot of things.
We had growing concerns regarding my sisters care and overall well being. Per the family's request, my sister was presented to her primary care physician for a physical. After examining her, the physician made a report to Adult Protective Services. My sister was malnourished, dehydrated, had a severe UTI, an eye infection in both eyes, bed sores and was not able to stand because of excruciating pain caused by extended immobility.
Prior to Covid restrictions, family was able to check in on her regularly. There were some concerns that were either brought to the staffs' attention by family members or in one case a report was made to DHEC. After Covid restrictions were put in place and family was not able to see my sister regularly, things went down hill quickly. Due to her mental condition, visitations at the window or through Facetime, were not an option. We relied on regular check-ins with the staff which became few and far between. We tried to call multiple times only to be disconnected or sent to a mailbox that was full.

My grandmother enjoys the place for the most part but is, like most patients, ready to go home. She has lived at the Brian Center for about a year without incident. She seems to enjoy the food for the most part and when she does not like the food, I believe that they provide alternatives. I would definitely give the Brian Center 5 stars if they were able to update their facility. Overall, I would say that the Brian Center is a relatively good place for family members to stay.
Unless the management is staffing philosophy changes please don't entrust them to care for your loved ones or yourself. Selecting the right skilled nursing facility can be critical to your speedy recovery. Brian Center Nursing Care/Sham located in Charlotte, NC, in Mecklenburg County, offers a variety of therapies and care services to help you regain your independence. As a result of the alleged fraud financial abuse and unnecessary staffing cuts, residents at the rehab center suffered badly and needlessly during the COVID-19 pandemic, the lawsuit states.
